so the black fade on the boots (and partly on the hands) is supposed to be a reference to the bubonic plague (which the plague doctors had exposure and were supposed to "treat" the patients who had this virus.) and so when someone catches the virus their feet turn black and fall off. it's brutal but I put it into this design
