you could mistake a deadly worm parasite as a grain of rice and eat it
you can choke on it or it may be dirty and have germs causing diseases
To be fair, It could also get into your eye and irritate it so much to the point where you have to get rushed to the ER and have surgery. Then one of the surgeons could accidentally cut something fatal in the eye which would cause you to bleed out and die.
